What you can expect
During your first visit with us, we will:
- Listen to your reason for coming to us
- Educate you about your options
- Make recommendations on how to proceed
- Explain how we charge for our services
- Completed Schedule of Assets form (Choose one of the following forms (PDFs)):
- Completed Information for My Attorney form (PDF)
- Any current legal documents you have
- Deeds to any real estate
- Original Will
- Death certificate, if available
- Funeral bill, if available
- Names and addresses for all Will beneficiaries and next-of-kin
- Any Trust documents
- Deeds to real estate
- Car Titles
- Most recent income tax return for the decedent
- Most recent statements for any of the following in the decedent's name (please include assets with a joint owner or named beneficiary):
- Bank Accounts
- Life Insurance Policies
- Investment Accounts
- Annuities
- Stock Holdings
- Bonds
- Pensions
- Outstanding debts (not including monthly expenses)
- Any other asset in the decedent's name
- Completed Medicaid Planning Information Form (PDF)
- Listing of all the income
- Deeds to any real estate
- Date of admission to a nursing home
- Dates of any nursing home stay of 30 days or longer in the person's history
- Copy of Power of Attorney
- Overall health status and prognosis
- History of any asset transfers in the last 60 months
