Living Will & Healthcare Power of Attorney
A Living Will is a legal document that says: “Do not hook me up. Just let me die naturally if I am in a permanently unconscious state and two doctors say I will not come out of it.”

The Ohio Living Will Declaration now includes your intent to make, or refusal to make, an anatomical gift (organ donation), and the Donor Registry Enrollment Form to be recorded with the Bureau of Motor Vehicles.

A Durable Power of Attorney for Health Care is a legal document that designates an attorney-in-fact to make medical decisions during the maker's lifetime if the maker is unable to do so.
