Death Certificates
A Death Certificate is necessary for each asset owned by a decedent that:
  • • Lists a beneficiary
  • • Is held as joint tenants with rights of survivorship
Following is a list of assets for which you will need a death certificate:
  • • Auto title (Surviving Spouse, Survivorship, and Transfer-On-Death)
  • • Life Insurance Policies
  • • IRAs/Employer Death Benefits
  • • Annuities
  • • Survivorship Bank Accounts/Savings Bonds/Stock/Brokerage Accounts/Deeds
  • • Pay-On-Death Bank Accounts/Savings Bonds
  • • Transfer-On-Death Stock/Brokerage Accounts/Deeds

You will need a death certificate for each asset transfer. For some, such as a bank account, you can request that the bank make a copy, but for others you will need to send an original that may not be returned.

Having several extra death certificates comes in handy for unexpected requests. You can get additional death certificates in Hamilton County (Ohio) by calling Vital Statistics.
